Overview: 

Under the direction of the Executive Vice President, Medical, the Physician delivers primary care commensurate with training and experience to all Avenue 360 patients. Delivers care appropriate for an outpatient, primary care FQHC with a multicultural, multilingual patient population.

Duties and Responsibilities:

· Examines, diagnoses and treats patients of all ages..

· Completes all EMR required fields after each visit including the e-signing of notes.

· Provides quality medical care according to Avenue 360 standards on quality assurance.

· Attends all internal provider meetings, committees, as necessary and/or required by Avenue 360.

· Performs all duties and services in full compliance with Joint Commission and funding source standards.

· May perform quality management/assurance activities.

· May participate in Mid Level supervision

· Participates in rotating on call coverage

· Performs miscellaneous job-related duties as assigned.

Educational and Job Related Requirements:

· Must be in possession of a valid license to practice in the State of Texas.

· Board certified or Board Eligible required.

· Current BLS Certification required.

· Must be in possession of a valid DPS and DEA licenses for practice.

· Have a thorough knowledge of the principles and practices of medicine commensurate with his/her training.

· Special interest in the area of community medicine and ability to relate to culturally diverse patients

· Bilingual in English and Spanish preferred.

Continuing Education and Requirements:

Participates in trainings required by the funding source and/or as required by licensure.
