Overview: This position’s focus is on identifying Ryan White (RW) clients who are on the cusp of or who have fallen out of care, for reengagement back into care by providing field-based services and clinic-based services. 

Duties and Responsibilities: 

  • Maintains a regular and predictable work schedule. 
  • Generate EMR reports to determine client Eligibility for services.  
  • Communicates with others (internally and externally) to provide, exchange, or verify information, answer questions, and address immediate issues of clients that are a barrier to continued or reengagement with care including direct linkage with Service Linkage Workers.   
  • Provide client Status updates on reengagement activities at Ryan White Grant Administration monthly meetings.  
  • Participates in multidisciplinary case staffing to ensure coordination of care and high-level of services provided to patients of the health center. 
  • Documentation of services provided in client record and required database(s) including data entry in EMR, CPCDMS, GPRA, etc., per reporting lines, within 72 business hours of occurrence.  
  • Performs quality management/assurance activities.  
  • Answers and returns patient telephone calls. 
  • Familiarity with HIV/AIDS, substance abuse and treatment and homeless services preferred.   
  • Objectivity and self-discipline to avoid emotional involvement in situations that are frequently emotional and high-charged.   
  • Other duties as assigned. 

Education, Experience, Licensure/Certification and Skills/Abilities Related Requirements: 

High School Diploma or GED. Six months of working with or volunteering with People living with HIV (PLWH). Must meet Standards of Care as prescribed by the respective funding source. Bilingual in English and Spanish preferred. 

 Continuing Education and Training Requirements: 

Participates in trainings required by the funding source.
