Resident Care Team Leader/PRN (Montrose)

Overview: 

Resident Care Team Leader Registered Nurse will assist in ensuring the general health and well-being of our residents by providing direct nursing care. This position is responsible for the direct nursing care of residents which encompasses planning, developing, organizing, implementing, and evaluating the care of residents to ensure that the highest degree of quality care and regulatory compliance is always maintained. 

Duties and Responsibilities: 

  • The resident Care Team Leader heads the Resident Care Team, which consists of the leader, a Resident Care Assistant and volunteer direct care givers. 
  • Participates in the direct and indirect care of residents and ancillary hospice services to residents’ family and friends. 
  • Performs all nursing duties based on standard of care established by State Board of Nursing. 
  • Oversees implementation of the plan of care, nursing interventions, direct nursing care distribution of medication, documentation of all care provided and bereavement for all residents. 
  • Inventory of controlled substances, documentation of dispensed controlled substances. 
  • Responsible for ensuring that the shift team meets all resident needs for personal care and ADL’s, activity, stimulation, mobility, safety, hydration, nutrition, elimination and post-mortem care for all residents. 
  • Coordinates and maintain clean and safe environment for residents, oversees meal and snack preparedness. 
  • Participates in Quality Assurance projects. 
  • Based in experience may be asked to assist as ‘on-call’ staff. 
  • Performs other duties as assigned by the Director of Nursing Services. 

 

Education, Experience, Licensure/Certification and Skills/Abilities Related Requirements: 

  • Currently licensed as a Registered nurse (RN) or Licensed Vocational Nurse (LVN) in good standing with the State of Texas. 
  • Licensed in the State of Texas. 
  • Is able to work to prescribed nursing standards including the physical demands of standing, walking, and moving most of the time without lifting restrictions. 
  • 1-year acute care experience required. 
  • 1 year Hospice and/or HIV/AIDS experience preferred. 
  • Ability to commit to and be flexible in unique care setting involving large number of volunteers. 
  • BLS Certification required  

 

Continuing Education and Training Requirements: 

CEUs required by Licensure and/or trainings designated by the funding source.
