Overview: The Director of Primary Care Services (DPCS) at Avenue 360 Health and Wellness provides direct patient care and under the direction of the EVP, Medical, this position is responsible for assisting with functional areas such as execution of quality activities, utilization management, and provider relations and communications in the pertinent Avenue 360 lines of service, including General Primary Care, Pediatrics and Obstetrics and Gynecology.
The DPCS will be assigned to and/or chairs various Avenue 360 committees at the discretion of Executive Vice President, Medical, including but not limited to Quality Assurance and Performance Improvement Committee (QAPIC), Pharmacy and Therapeutics Committee and Risk Management Committee.
Duties and Responsibilities:
- Collaborate with other providers and members of the delivery system to continuously improve the health of the population that we serve.
- Support the transition of the clinical practices into Patient Centered Medical Home as well as value-based care via integrated practice units when possible.
- Assist in the oversight and collaboration of all medical, pharmaceutical, oral health and behavioral health outpatient programs in the pertinent regional clinics including Care Management support, utilization review and quality targets.
- In conjunction with the Executive Vice President, Medical, interpret data-based utilization analysis, including cost trending in key areas (pharmacy, after-hours use, ED visits by patients with ambulatory sensitive conditions, high risk uncontrolled chronic disease trends etc.).
- Function as Super User for the EMR in the pertinent regional clinics.
- Participate in information sharing, process improvement discussions, and problem-solving meetings with clinical, administrative and health equity teams.
- Share responsibility to manage innovative projects and track their progress to completion.
- Assist in developing and implementing system-wide, standards of clinical care, medical quality assurance/management programs, practice guidelines and protocols, interdisciplinary collaboration, plus policies and procedures related to the delivery of patient care and patient experience.
- Assist in provider recruitment efforts to ensure clinics in the line of service are properly staffed with an effective and balanced mix of qualified providers.
- Collaborate with quality management, compliance, and human resources teams in evaluating provider performance, privilege maintenance and implementation of disciplinary actions when necessary.
- Aid in the development, implementation, and maintenance of treatment and prevention services, in accordance with federal, state, and local community health regulations and standards.
- Stay abreast of current performance improvement processes and help implement and oversee clinical performance improvement in the pertinent region.
- Foster an environment that promotes the Avenue 360 Quality Assurance and Improvement Program and is responsible for removing barriers in the pertinent regional clinics, in dyad partnership, with the site managers to achieving quality in medical care and for reporting to internal and external committees and entities, as required.
- Participates and takes proactive role in customer service related issues and ensures to adhere to the values created by Avenue 360 by continually striving to improve and provide quality patient care, treating everyone with respect and dignity, promoting teamwork and cooperation to create an environment that maximizes efficiency and satisfaction, encouraging open communication and adhering to all established ground rules.
- Provides direct clinical services in the area of board-certified medical specialty in accordance with the highest applicable standards of medical and professional practice and in full accordance with health center protocols, policies and Director of Primary Care Services job description.
- Provides supervision to non-physician providers in the allowable specialties according to the state medical board rules and regulations.
- Act in a manner to further the Avenue 360 patient care and community care interests including but not limited to patient centered team-based care, supporting the Strategic Plan, meeting program requirements, communicating effectively up and down reporting lines, and working with the leadership to address the needs of the patients and the organization.
Educational and Job-Related Requirements:
- Must be in possession of a valid license to practice in the State of Texas.
- Board certified in Family Practice required.
- Must be in possession of a valid DPS and DEA licenses for practice.
- Current BLS Certification required.
- Have a thorough knowledge of the principles and practices of medicine and allied services commensurate with his/her training.
- Demonstrate a special interest in the area of community medicine.
- Ability to relate to culturally diverse patients and community.
- Experience in HIV/AIDS care is preferred.
- Bilingual in English and Spanish preferred.
Continuing Education and Requirements:
Participates in trainings required by the funding source and/or as required by licensure.
