Overview: Under the supervision of the Executive Vice President of Medical, and through a team approach the ideal candidate will: (1) provide leadership in, and responsibility for, all aspects of the development and execution of the Avenue 360 Quality Improvement (QI Program), Quality Assurance and Performance Improvement (QAPI Plan) and Population Health Program; and (2) works on the strategies to enhance the ability of Avenue 360 clinics and Health center to deliver high quality health care that lead to positive patient-centric health and service outcomes. (3) This position also provides direct patient care.
Duties and Responsibilities:
- Participates as a key member of health center clinical leadership team in the development, implementation and evaluation of population health, quality and practice transformation strategies.
- Responsible for the day-to-day operations and implementation of Avenue 360 Quality and Population Health initiatives and policies.
- Provide technical assistance, coaching and trainings to Avenue 360 staff in pursuit of the Quadruple Aim through the use of QI theory (e.g. IHI Model for Improvement), methods (e.g. PDSA cycle).
- Applies population health concepts and implements appropriate quality improvement methodologies to assure highest standards of population health and quality improvement infrastructure and performance. Collaborates with the EVP, Medical to provide oversight of FTCA QA/QI compliance.
- Oversee Avenue 360 data staff to enhance systems and tools (e.g. scorecards, dashboards) to collect, validate, analyze, report on key quality and performance metrics, and improve Avenue 360 clinical and operational data with respect to clinical reporting requirements and standards as established by funders and in alignment with Avenue 360 QAPI plan.
- Convene and manage regular meetings and activities of the Avenue 360 QAPI committee under the direction of Avenue 360 Chief Medical Officer.
- With the EVP, Medical, and the Avenue 360 directors, participate in the development, management and implementation of the development of QI and other health care policies, procedures and protocols for Avenue 360 programs and service lines while ensuring their consistency with applicable laws, regulations, program expectations, and professional standards.
- Provides direct and ongoing outpatient clinical care for assigned patients and documents all client care in the EMR as appropriate in a timely manner. Assures a safe care environment by instituting appropriate control measures.
- Other duties as assigned by EVP, Medical.
Educational and Job-Related Requirements:
- Masters in nursing, Health Care Administration, Informatics or MBS preferred.
- Current Registered Nurse license required.
- Quality Management experience required.
- Familiarity with ambulatory EMR systems required.
- Experience with survey process (e.g. Joint Commission, FTCA, HRSA, etc.).
- Basic Life Support (BLS) Certification
- Must have strong organizational and project management skills.
- Must have excellent judgment and creative problem-solving skills.
- Highly organized and great attention to detail
- Must have the ability to work with minimal supervision.
- Must be able to work in a fast-paced, high-intensity work environment.
Continuing Education and Requirements:
Participates in trainings required by the agency and funding source. Must secure CEUs annually to maintain licensure and provides copies of CEUs to HR.
